Boston Senior Home Care provides Care Management services that include:
- Conducting in-home assessments
- Creating a personalized care plan
- Arranging the implementation of in-home services
- Monitoring and modifying services as needed (ongoing)
Our funding
Boston Senior Home Care is a contractual partner with the Executive Office of Elder Affairs (EOEA) in administering the Home Care program in the Commonwealth.
The Commonwealth contracts the local organizations such as Boston Senior Home Care to manage the delivery of services. Smaller community based organizations will have a better understanding of local needs and service providers, resulting in higher quality and more cost effective services, better response to client needs, and greater accountability for quality of services.
