The services are intended to encourage independence and to ensure dignity. The Home Care program also supports families caring for elders at home in order to encourage and to relieve and address the stress of caregiving.
Boston Senior Home Care Case Managers conduct comprehensive needs assessments to determine eligibility for the Home Care Program and other programs as appropriate. Once an elder is admitted to the program, a service plan is developed for client. Case Managers monitor client services and the client's needs on an ongoing basis.
Eligibility
Eligibility for the Home Care Program is based on age (60 years of age or older, or under age 60 with a diagnosis of Alzheimer's disease), financial status, and ability to carry our daily tasks such as bathing, dressing and meal preparation.

- Homemaker services, including light housekeeping and meal planning and preparation
- Personal care services to help with bathing, dressing, toileting, eating, and other activities of daily living
- Adult Day Health services to provide health care, supervision, and socialization
- Dementia Day Care to provide a structured supervised setting for seniors with cognitive disabilities
- Home Health services, including skilled nursing care, home health aide, and therapies
- Home Delivered Meals, for homebound seniors unable to attend congregate meal sites
- Transportation
- Medication assistance
- Food shopping and delivery services
- Laundry
- Chore services to maintain client's home or yard
- Companion services, to accompany client on errands, on medical appointments, or within the client's home
- Adaptive housing to equip or modify client's home
- Personal Emergency Response System (PERS), for medical emergencies
