Home Power services make it possible for disabled individuals aged 22 and over, including seniors, who live in an assisted living residence or specially designated public or subsidized housing to continue living safely at home.

Eligibility
- Must be MassHealth (Standard or CommonHealth) eligible
- Age 60 or older, or 22 or older and chronically disabled
- Lives in designated public or subsidized housing or an assisted living facility that participates in GAFC
- Has functional or cognitive impairment that prevents the client from living safely and independently in the community
- Needs daily assistance with one or more personal care tasks such as bathing, dressing, toileting, meal preparation, and/or moving about
- Has a physician's statement of client's condition
- Requires and accepts personal care assistance 5-7 days per week
Services
- Personal care and homemaking services (5-7 days per week)
- Alternating monthly home visits by a registered nurse or case manager
- Home Power services can work in collaboration with a certified home health agency (i.e. visiting nurse and/or therapist may continue to come)
- Based on client's needs: Adult Day Health (2-5 days per week), home delivered meals, grocery shopping, homemaking, laundry, and transportation arrangements
- Personal Emergency Response System
Once admitted to the program
The Home Power (GAFC) Nurse and Case Manager make alternate monthly home visits for case management and oversight. The personal care homemaker visits client daily to assist with personal care homemaking according to the nursing developed care plan.
