Boston Senior Home Care co-manages one program at
Codman Apartments located
at 784 Washington Street in Dorchester, and is co-piloting a second program at
Blackstone Apartments located at 33 Blossom Street in Beacon Hill.
Codman Square Apartments
The Codman Square Apartment Supportive Housing program is a joint venture between Boston Senior Home Care, the Boston Housing Authority, and the Visiting Nurses Association of Boston. The site provides support to approximately 100 residents, from many diverse backgrounds. Special program features include:
- Boston Senior Home Care on-site Service Coordinator; who is available to provide information, referral and assistance to all residents, as well as ongoing case management and support services to those residents who are Boston Senior Home Care clients
- Monthly wellness seminars provided by the Visiting Nurses Association of Boston, includes
- Blood pressure and cholesterol screenings
- Nutrition workshops
- Stress management
- Women's health issues
- On-going exercise and yoga classes
- On-site computer lab
- Congregate meal program (provided by Kit Clark Senior Service)
Blackstone Apartments
The Blackstone Apartments Supportive Housing program is joint project between Boston Senior Home Care and Massachusetts General Hospital (MGH)'s Senior HealthWISE, and State Street Development Corporation. The site was opened in 2007 and serves approximately 150 residents. Special program features include:
- Boston Senior Home Care on-site Service Coordinator; who is available to provide information, referral and assistance to all residents, as well as ongoing case management and support services to those residents who are Boston Senior Home Care clients
- Wellness programs, such as
- Nursing clinics
- Blood pressure screenings
- Upcoming Exercise Classes
- Workshops on topics such as managing chronic health conditions, and fall prevention
- Ongoing access to the services of a Social Worker and Resource Coordinator provided by MGH
- Social activities coordinated with the Property Manager
- Twice weekly coffee and teas
- Monthly movie screenings
- Weekly arts and crafts classes with instructor from the West End Senior Center
- Holiday events
- Classes on such topics as food stamps and other vital social services
